This form is required for all learners, both under age, or adults, to be completed regarding entrance into the 22 Credit Minimum Graduation Pathway. The form advises you about the possible limitations incurred by graduating under this Minimum Pathway.  Once approved, you will meet with your adviser to determine your path forward.
